- Fill out the top portion (Part 1) of the form. Please make sure you provide all required information and print clearly.
- Bring the claim form to your veterinarian to fill in Part 2 of the form and sign.
Note: Ask your veterinarian if they are using EVE (Online insurance claims filing for veterinary hospitals and clinics.)
- Sign and date your claim form before you fax or mail it in.
